Peaceful Birth Midwifery  Privacy Statement & Informed Consent

Practice Philosophy

Birth is a natural and beautiful experience, which can empower women throughout their lives.  It is a rite of passage and many women realize their greatness, power, and strength as they experience this journey into motherhood, supported emotionally and physically, in a safe and peaceful environment. Birth is a normal life experience ~ not an illness. It is a profound emotional, spiritual and physical experience which has significant meaning. The birth process works best when unnecessary interventions are avoided. It is every woman’s responsibility to educate herself regarding all aspects of her health and we facilitate that process. The mother is the primary decision maker regarding her care. Evidence-based care and continuity of personalized, individualized care is one of the most important advantages of midwifery care. Midwives encourage family centered care. We believe that every woman can find PEACE as she becomes educated, informed and empowered to make choices surrounding the birth of her child.

Midwifery Care, Services and Responsibilities

We provide fertility counseling, prenatal care from conception to birth, postpartum and newborn care until 6 weeks after birth, and well-woman care for non-pregnant woman. Appointments are held at my home office on a task-based/evidence-based schedule or traditional schedule every 4-6 weeks until 28 weeks of pregnancy, and then every 2 weeks until 40 weeks, appointments each week until birth. Postpartum care includes a home visit at 24-48 hours, and office appointments at 1-2 and 4-6 weeks.

Various testing is offered in pregnancy, these tests are available either in our office or by referral to another provider. The testing we currently recommend is an obstetric panel (especially if blood type is not known) or hemoglobin level near the beginning of pregnancy and a recheck of hemoglobin levels with a finger stick in the later stages of pregnancy. Other testing will be recommended as needed based on conditions and symptoms or at your request.

After 37 weeks of pregnancy we will be on-call for you until you give birth, or we will provide appropriate backup. If we are at another birth, we will send an assistant midwife until we can arrive.

Student midwives will provide care as assistants, and/or primary midwives under supervision, and will review de-identified client charts for educational/certification purposes.

Experience and Education

Amy is a Licensed Midwife in the State of Utah and a Certified Professional Midwife through the National Association of Registered Midwives, she is also a certified preceptor (trainer) for student midwives. To receive these certifications, she apprenticed with a Certified Nurse Midwife for 2 years in Las Vegas, NV, passed a written examination of academic knowledge and submitted proof of birth experiences and validation of a wide variety of clinical and emergency skills. She currently holds CPR and Neonatal resuscitation certifications. She is not a Certified Nurse Midwife or Physician. She has been attending births since 2015 as a student, doula and midwife.

Medical Transfer Plan

When medical services are needed that cannot be provided, we will refer you to other care providers. Because of the long-lasting effect of your birth experience, your desires will be valued, second only to safety of you and your baby. If you transfer to the hospital during labor, we will contact a known provider if possible or we will use the provider on call at the hospital to which you transfer. Transfers occur by private car, or by ambulance if needed. You may at any time change your maternity care provider or consult with another health care provider.

Legal Concerns

Direct-entry midwifery and home birth are legal in Utah. As required by Utah law, we will file a birth certificate with the state for your baby. Upon request, we can provide the rules governing licensed Direct-entry midwifery practice including a list of conditions indicating the need for consultation, collaboration, referral, transfer or mandatory transfer, which are the guideline with govern this practice.

Malpractice Insurance

We are not covered by malpractice or professional liability insurance.

Parents' Responsibilities

Please respect our time and training, be on-time to appointments, email questions that can be answered within 24 hours, call during appropriate times (12 p.m.- 5 p.m.) for non-urgent matters, fully disclose your birth plans and most importantly respect our recommendations for health during pregnancy & during birth in case of urgent transfer to medical care.

Be completely honest regarding previous medical and pregnancy history, current exposure to disease, infection or illness, medications you are taking including herbs, prescriptions and overt the counter medications, recreational drug use and contact with other care providers.

Be involved in your own care and self-education; for first time parents this usually requires attending a formal birth education class and having a doula (or experienced birth support person during labor). Make dietary changes we deem necessary and obtain the best quality foods during pregnancy.

Grievance Process and Peer Review

If you have a complaint regarding our services, your first responsibility is to address it with us directly, in person, by phone, or in writing. You agree NOT to post to social media or any website any grievance, complaint or other negative review. We will never discuss you nor your situation/birth (other than with expressed permission) and expect the same courtesy. If you are not satisfied with our response, you may file a complaint with the Utah Department of Professional Licensing, Utah Midwives Organization and/or with the North American Registry of Midwives.

To assure quality care and continually improve our services, we periodically review difficult or unusual cases through the Utah Midwives Organization's peer review/case study process, and/or within our own practice's peer review. All personally identifiable information is deleted to protect client confidentiality.
